We understand the unique considerations of medical surgical hospitals as they care for medically complex patients. Today’s complicated healthcare environment places tremendous pressure on providers to identify the most appropriate care setting for each patient. From our evaluation process to patient transfer through discharge from our hospital, our case management and medical team is dedicated to the medically complex patient so that community hospitals can focus on their broader patient base.
Our team of clinical liaisons and case managers work hand in hand with referral sources and family members to facilitate a smooth and positive admissions process. After receiving an initial inquiry, one of our professionally trained clinical liaisons will promptly conduct a patient evaluation and respond back to the referring party in a timely manner.
