Admissions The process is easy. Simply have a physician write an order for "LifeCare Evaluation" for transfer or admission. Upon confirming the physician's order and consent for clinical evaluation, an experienced Clinical Nurse Liaison will meet with the patient and family, to educate them about the LifeCare program and to complete a comprehensive medical evaluation. We highly encourage the family to ask questions and to schedule a time for a tour of the facility. Upon acceptance, the intake coordinator will coordinate with the family, referring facility and the insurance company to facilitate a smooth transition to our hospital.
