Insurance Plans
This is our best effort to list those managed care plans which will allow use of your "in-network" acute care hospital benefits. We update this list regularly however due to the frequency with which new plan products (i.e., PPO, POS, HMO, etc.) are introduced we are unable to include the specific plan products. Please contact your employer benefits specialist or the health plan to verify specific plan/product participation status including your health plan benefits pertaining to the provision of complex acute care hospital services. Be sure to have your plan identification numbers available.
Some plans have given us permission to link directly to their Internet site. Simply place the cursor over the name of the plan below and "click." You should be able to view the plan's home page and in most instances be able to navigate from the plan's home page to its provider directory. You should be able to locate LifeCare Hospitals of Wisconsin as a "participating provider” depending upon the plan product you select.
LifeCare Hospitals of Wisconsin also participates in a wide array of Medicare Advantage (MA) programs. Your MA plan’s website should give you similar network participation information as pertains to our hospital. Some additional considerations:
- There are MA non-network” or open access plans (such as Private Fee For Service [PFFS]) where we are deemed to be a participant by virtue of our Medicare certification. In most cases, no pre-authorization is required.
- There are MA “network” products (such as PPOs and HMOs) with which we may or may not be contracted. Despite representations of enhanced Medicare benefits, some of these plans do NOT cover our care or will provide coverage only in very limited circumstances.

You are welcome to contact our Intake office where we will attempt to advise you as to your access to our care based on the Medicare Advantage plan in which you currently participate or are contemplating participation.
